Coordination of Benefits

Staff who are enrolled in a Froedtert Health medical plan must complete a Coordination of Benefits form on an annual basis. If Exceedent does not receive a completed form, claims may be denied until the form is received.

Authorized Representative Form

All members over the age of 18 must complete an Authorized Representative Appointment Form in order to inquire on any personal health information, appeal, obtain benefit information, etc. Be sure to select the function(s) that the representative is being authorized to do. You can select more than one representative and choose the same or different functions. The representative may be an individual or an organization. Once appointed, the designated Authorized Representative is valid indefinitely until the member directs Exceedent differently. Note: This form must be completed for Exceedent to talk to you about your spouse or over aged dependent’s accounts.
